Rally is an app that brings together a suite of clinical and wellness programs, available to you at your fingertips. By participating in Rally you can earn coins, which can be exchanged for prizes.
NurseLine is a 24 hours a day, 7 days a week, telephonic service that puts you in touch with a registered nurse. When you call, a registered nurse can help you: Choose the appropriate medical care for your needs— whether that is an emergency room, doctor visit or self-care; Understand your diagnosis and explore diﬀerent treatment options and outcomes; Achieve a healthy lifestyle and get tips on nutrition, exercise, health screenings and immunizations; Learn how to save money on prescriptions, and take your medications safely and avoid interactions and more!
The SJS program gives you access to specialized nurses to help meet your specific needs across the continuum of care, from early spine and joint pain onset through treatment and beyond. In addition to the nurse, SJS gives you access to the Centers of Excellence (COE) network. This is a network of top surgeons, hospitals and ambulatory centers that have been recognized for quality. SJS empowers members to actively participate in decisions about their health care and treatment options.
If you or a loved one has cancer, take advantage of the Cancer Support Program. If you’re preparing for cancer treatment or have already started, an oncology nurse can help you navigate treatment options and help you find a network provider from a high-quality Centers of Excellence (COE) facility.
Transplant Resource Services:
If you need help deciding where to go for a transplant, this program may be for you. Based on your needs, a special transplant nurse can refer you to a medical center from our Centers of Excellence network. Plus, your nurse will work with your doctors to help coordinate your treatment before and after a transplant.
Heart Failure Support Program:
If you find it difficult to manage your heart condition, the Heart Failure Support Program may be right for you. Connect one-on-one with a personal nurse who will work with you over the phone to help you: Understand your condition and get information about cost-effective and appropriate tests and procedures; Manage your condition through daily monitoring of weight and symptom changes; Understand the right medications for heart failure and monitor your prescriptions; Coordinate care with your doctor and reduce unnecessary hospital stays.
Diabetes Support Program:
If you find it difficult to manage your diabetes, the Diabetes Support Program may be right for you. Connect one-on-one with a personal nurse who will work with you over the phone to help you understand your treatments, tests and procedures; reduce risk factors, such as high cholesterol, high blood pressure and excess weight; and help ypu establish exercise and weight loss goals to maintain a healthy lifestyle.
Coronary Artery Disease Support Program:
If you find it difficult to manage an ongoing heart condition, the Coronary Artery Disease Support Program may be right for you. The program will connect you with a personal nurse who will work with you over the phone to help you improve your health.
Healthy Pregnancy Program:
If you’re thinking about having a baby or have one on the way, the Healthy Pregnancy Program is here to provide information and support. When you enroll in the program, you’ll be able to work with a maternity nurse who can help you choose a doctor or nurse midwife, find a pediatrician or other specialist, help you manage your physical and emotional health before and after you baby is born and more!
Neonatal Resource Services (NRS):
Having a baby that needs extra care can be scary and stressful. But Neonatal Resource Services (NRS) is here to provide information and support. When you enroll in the program, you’ll be able to work with one of our neonatal nurses, who will answer your questions and help you with things like: making a plan for bringing baby home, lining up home services and equipment, putting you in touch with local services and resources and more. This may be a challenging time, but we want you to know we’re here to help you.
